The Application for Admission provides basic contact information as well as health and educational information about our prospective students. If you interested in applying to Cedar River Academy please download and complete this form, and return to Cedar River Academy for consideration.

The Enrollment Agreement is the financial contract for tuition at Cedar River Academy and should be turned in with the Application for Admissions.
